Performance Live Scan is operated by Performance Professional Services. Together with Performance Surety Bonds and Insurance Services, we provide mobile live scan fingerprinting, passport photos, notary public services, surety bonds, and more.

Walk-ins are always welcome during our regular business hours.

Please remember to bring your Live Scan form to our office. If you need notary supplies, bring your Certificate of Authorization.

Performance Live Scan Fees

$15 – Live Scan Rolling Fee for Notaries and Contractors
$25 – Live Scan Rolling Fee for Others
$10 – (2) Passport Photos
$25 – Minimum trip charge

Our Staff

Performance Live Scan is owned and operated by Sam Shoemaker. Sam is a University of Iowa graduate, California insurance agent, certified fingerprint roller, and California notary public.

Dayna Monroe-Main is Sam’s assistant and is also a California notary public.
